Waiver/Release Form

All players must have a completed Waiver/Release form before they are permitted to participate in any Warriors program. Please print and complete the form to submit to the coaches prior to your childs first event. There may be some forms on site if you have difficulties printing the document, please let your coach know you’ll need one.
